📷 Use of Image and Likeness
By submitting an application to the Children's Funeral Expense Program, the legal parent or guardian may voluntarily grant SHHF permission to use the deceased minor’s image or likeness (such as photographs or graphics) for the following purposes:

This permission is granted only if the parent, grandparent, or legal guardian expressly consents by checking the appropriate box during the application process.

SHHF requires a valid copy of the death certificate before any funds are disbursed to the funeral home. This document helps verify eligibility and ensures assistance reaches the appropriate parties with dignity and transparency.
